Doctor: Education is important to prevent blindness due to glaucoma

Why do you get treatment?', why do you have surgery if you still can't see?... It's not a cure to get back to the way it was before, but to prevent it from getting worse
- Doctor from Cicendo Eye Hospital Dr Elsa Gustianty said education about glaucoma is important for the public so that blindness due to glaucoma can be reduced.
"The aim is to increase public awareness of glaucoma so that they are encouraged to get checked," he said in "Prevent Blindness Due to Glaucoma" broadcast by the Ministry of Health (Kemenkes) in Jakarta, Friday.
Elsa said that globally 80 million people suffer from this condition. In Indonesia, said Elsa, glaucoma is the second most common cause of blindness. The most common cause is cataracts.
Apart from early detection, he continued, education is also important so that patients want to come to the doctor for control.
According to him, the disease has a psychological impact. When someone is diagnosed with glaucoma, he said, there are patients who do not want control because they feel that their efforts are in vain, because there will be no change.
"'Why do you have treatment?', why do you have surgery if you still can't see? In fact, our effort is to maintain it. It's not about treating it back to the way it was before, but preventing it from getting worse," said Elsa.
He said glaucoma can attack anyone, but cannot be treated, because the condition is the effect of a degenerative process, like graying hair.
Elsa said there are risk factors that can accelerate and worsen glaucoma, namely age, vascular disease including hypertension and diabetes, use of steroids, and family factors.
On that occasion he explained that a number of steroids could create a feeling of comfort, for example dexamethasone, prednisolone and prednisone. When used, he said, red eyes turn white and inflammation immediately disappears, so it is often misused by people who have allergies.
He explained that a number of people who usually wear contact lenses use these drugs just to get rid of itching in their eyes, until without realizing it, after years of use, the pressure in their eyeballs is high.
In glaucoma, he said, pressure in the eye is caused by water that the eye cannot circulate into the blood vessels. Due to excess water production, he said, then added to the obstacles in the distribution, the volume of fluid increases until it finally presses on the nerves and blood vessels behind the eyes, until finally the nerves slowly die.
He said that nerves have their own unique characteristics, so that when one nerve dies, those around it also die. Therefore, he said, glaucoma is progressive.

- Dokter dari Rumah Sakit Mata Cicendo Dr Elsa Gustianty mengatakan edukasi tentang glaukoma penting untuk publik agar kebutaan akibat glaukoma dapat ditekan.
"Tujuannya adalah meningkatkan awareness atau kesadaran masyarakat terhadap penyakit glukoma sehingga mereka terdorong untuk memeriksakan diri," ujarnya dalam "Cegah Kebutaan Akibat Glaukoma" yang disiarkan Kementerian Kesehatan (Kemenkes) di Jakarta, Jumat.
Elsa menyebut secara global 80 juta orang mengidap kondisi itu. Di Indonesia, kata Elsa, glaukoma adalah penyebab kebutaan terbanyak nomor dua. Adapun penyebab terbanyak adalah katarak.
Selain untuk deteksi dini, lanjutnya, edukasi juga penting agar pasien mau datang ke dokter untuk kontrol.
Menurutnya, terdapat dampak psikologis yang ditimbulkan penyakit itu. Ketika seseorang divonis terkena glaukoma, kata dia, maka ada pasien yang tidak mau kontrol karena merasa usahanya sia-sia saja, karena tidak akan ada perubahan.
"'Ngapain ya diobatin?', ngapain ya dioperasi kalau tetap tidak bisa lihat?. Padahal kita itu usahanya adalah mempertahankan. Memang bukan mengobati menjadi kembali seperti dulu, tapi mencegah supaya tidak lebih buruk," ucap Elsa.
Dia menuturkan glaukoma dapat menyerang siapa saja, namun tidak dapat diobati, karena kondisi tersebut adalah efek dari proses degeneratif, layaknya rambut yang memutih.
Elsa mengatakan terdapat faktor risiko yang dapat mempercepat dan memperparah glaukoma yaitu umur, penyakit vaskuler antara lain hipertensi dan diabetes, penggunaan steroid, serta faktor keluarga.
Dalam kesempatan itu dia menjelaskan sejumlah steroid dapat menciptakan rasa nyaman misalnya dexamethasone, prednisolon, dan prednison. Ketika dipakai, katanya, mata merah menjadi putih dan radang langsung menghilang, sehingga kerap disalahgunakan orang yang punya alergi.
Dia menjelaskan sejumlah orang yang biasa memakai lensa kontak memakai obat-obatan itu hanya untuk menghilangkan gatal di matanya, hingga tanpa sadar setelah pemakaian bertahun-tahun, tekanan pada bola matanya tinggi.
Pada glaukoma, kata dia, tekanan di mata disebabkan oleh air yang tidak dapat disirkulasi mata ke pembuluh darah. Karena produksi air berlebih, katanya, kemudian ditambah dengan hambatan di penyaluran itu, volume cairan meningkat hingga akhirnya menekan syaraf-syaraf serta pembuluh darah di belakang mata, hingga akhirnya syaraf-syaraf itu mati perlahan.
Dia menyebut syaraf memiliki keunikan sendiri, sehingga ketika ada satu syaraf yang mati, maka di sekitarnya juga ikut mati. Oleh karena itu, katanya, glukoma bersifat progresif.
